Using your insurance
Insurance participation can vary by the specific plan offered by your employer or carrier. Please contact your insurance company to confirm your plan, then compare it with the lists below. This is a general reference and cannot be guaranteed to include every plan.
If your plan appears on our participating list and you would like to schedule, please call us with the member or subscriber ID, group number, policyholder name and policyholder date of birth ready for our scheduling team. We accept dental plans only, not medical plans that include dental benefits.
